Section 1 — Initial Incident Description / Описание инцидента
On 1987-04-02 at approximately 09:15, Facilities Coordinator V. ORLOV received a complaint from personnel in Break Room B (Room 114) regarding the percolator unit installed in that room (Serial No. CF-1974-0041). The complaint stated that the unit had ceased to produce coffee at the expected rate and that an unusual odor had been observed. Upon inspection, CF-1974-0041 was found to be operational but producing output at approximately one-third the normal volume. The heating element was functioning within normal parameters. The water reservoir appeared full. The output was the correct color and consistency. The odor observed by personnel was described by this officer as "burnt" and "slightly sweet." No smoke was present. No visible damage was found. Unit was unplugged as a precautionary measure. Maintenance request submitted to procure replacement heating coil (Part No. HE-7, standard). Personnel were directed to use the percolator in Break Room A (Room 108) in the interim. No injuries. No property damage. No further action anticipated at time of filing.

Replacement heating coil (Part No. HE-7) was installed on 1987-04-04 by Technician M. PAPADAKIS. Unit was returned to service at 14:00. By 14:45, three (3) personnel from the adjacent offices had returned to Room 114 to report that the coffee produced by the repaired unit tasted "wrong." When asked to elaborate, personnel used the following descriptions (recorded verbatim):
— "Like something that used to be coffee." This officer tasted a sample at 15:10. The coffee appeared normal. The taste was within normal parameters in all identifiable respects. This officer noted no anomaly. No further action taken. Personnel complaints logged for the record. The smell observed on 1987-04-02 had not returned as of the time of this note.

Four additional complaints received 1987-04-07 through 1987-04-08. Unit CF-1974-0041 has been producing output consistently since repair. The complaints do not concern volume, temperature, or appearance. The complaints concern the taste. Personnel are now reporting secondary effects. This officer has compiled the following from written complaint slips (Form MED-3, Minor Discomfort):
F7-0023 (KOWALSKI, D.) — Reports temporary difficulty recalling the word for "window." Duration: approximately one hour. Resolved without intervention. Medical review requested. Unit CF-1974-0041 removed from service pending review. Water supply to Room 114 tested; results normal.

Medical Division reviewed complaint slips and conducted brief interviews with personnel F7-0023, F7-0055, and F7-0088 on 1987-04-11. Findings are as follows:
All three personnel presented no measurable physiological abnormalities. Medical Division recommends no action. Medical Division has appended a note stating: "The coffee maker is a coffee maker. There is no reason to treat a coffee maker as anything other than a coffee maker. The coffee maker makes coffee. This is what coffee makers do." This officer found the appended note unusual in tone and has included it here for the record. It was not solicited. Unit CF-1974-0041 remains out of service. This report is hereby reclassified from CONFIDENTIAL to SECRET by order of ████████████████, effective 1987-04-19. All copies below classification SECRET are to be recalled and destroyed. Grounds for reclassification: Unit CF-1974-0041, which had been secured in Storage Closet 114-B since 1987-04-09, was found operating on the morning of 1987-04-17. The storage closet had been locked. The unit was not connected to any power source. The unit was producing coffee. Approximately 0.4 liters of coffee was found in the carafe. The coffee was hot. The coffee was fresh. The coffee was, by all measurable criteria, coffee. Technician M. PAPADAKIS, who discovered the unit, did not immediately report the finding. When interviewed, he stated that he had not thought to report it. He stated that he had, in fact, poured himself a cup. He could not say whether he had drunk it. Oversight of this incident is transferred to ████████████████ Division effective immediately. Facilities and Maintenance personnel are no longer the primary reporting officers for this matter. Personnel V. ORLOV and T. HENRIKSEN are to be briefed on new handling protocols under separate cover. Personnel M. PAPADAKIS has been placed on administrative leave pending review. M. PAPADAKIS expressed no objection. M. PAPADAKIS expressed nothing. M. PAPADAKIS smiled and said: "Of course. That seems right." Personnel are advised that this is not how M. PAPADAKIS normally speaks.

Unit CF-1974-0041 was examined by ████████████████ Division specialists between 1987-04-22 and 1987-04-30. The following is a partial summary. Full findings are contained in Report ████████████████, access to which requires clearance level ███████. Physical examination: The unit is structurally identical to the model as manufactured. No modifications. No unknown components. The power cord, which was confirmed to be unplugged and later confirmed to be absent, was found reattached when specialists arrived. The cord was the original cord. The outlet it was plugged into does not appear on the building's electrical schematics. Output analysis: Coffee samples collected on 1987-04-22 were submitted for chemical analysis. Analysis confirmed the samples to be coffee. Analysis further confirmed the presence of ████████████████ compounds not produced by any known roasting or brewing process. Specialists noted that the compounds were not harmful. Specialists noted that describing the compounds as "not harmful" was the most accurate available description, and also that it was not entirely accurate.
Behavioral observations: NOTE: Specialist ████████████████ submitted a supplemental observation on 1987-04-28. The note read, in its entirety: "I think it is waiting. I do not know what it is waiting for. I do not think the wait is long by its measure." The specialist subsequently took four (4) weeks of medical leave. No further information available.
